Faurenis keeps the new-patient form short on purpose. You can always add the long medical history later from the chart.
- Legal name, date of birth, and contact channel — required.
- Insurance and emergency contact — optional, prompted on second visit.
- Medical history flags — taken via the patient-intake form, not the chart.
